Before the staff at Children's Hospital of Pittsburgh Foundation can contact a patient family, the family must sign a HIPAA Release Form (PDF). Please ask the familyto complete this form and then send it through interoffice mail to Carol Ashby, Director, Major Gifts, at the Foundation.
